1. Please make sure your file name includes the file extension
2. Please make sure your file names DO NOT have spaces or special characters (ie. *, &, $, \, /, @, etc.)
3. After you click the 'upload' button, please wait for the acknowledgement page to confirm that file was successfully uploaded
